Using CO2 lasers for scar removal on darker skin types (Fitzpatrick IV–VI) is considered an advanced procedure that is safe & effective, but it carries a higher risk of complications if not performed correctly. I have over 15 years of experience using Lasers & all different Modalities on all skin types. I have effectively treated thousands of clients & am highly skilled due to a very high repetition of difficult cases, scars & frequently participate in more advanced learning as well as performing my own trials. I use the CoRE2 Co2 Laser by Candela. It is the most advanced technology with a very wide range of uses due to the wide range of treatment parameters it can accommodate. For clients the most annoying part of the process and the primary concern is Post-Inflammatory Hyperpigmentation (PIH)—which is a temporary darkening of the skin—due to higher melanin levels, which happens normally to almost all clients that I treat. This temporary pigmentation goes away at the end of your sessions. You must be prepared for the aftercare & proper healing care after all of your sessions for the fastest results and best results.
Here are the frequently asked questions regarding CO2 laser scar removal on darker skin:
Safety and Risks
-
Is CO2 laser safe for dark skin?
Yes, but fractional CO2 technology is required, not older fully ablative lasers. An experienced provider must adjust settings (lower fluence, lower density) to prevent permanent pigment damage or scarring. - What will happen after my sessions?
- Post-inflammatory hyperpigmentation (PIH): Dark spots appearing 1–2 weeks post-treatment.
- Dark MicroScabbing: Sandpaper like Micro-Scabs caused by the controlled Micro-Injury the laser causes
- Keloids/Hypertrophic scars: Will become softer and turn pink in time and eventually will flatten and turn back into normal skin.
-
Will I definitely get hyperpigmentation?
It is common but not permanent. It can be managed with proper pre- and post-treatment care.
Procedure and Preparation
-
What is the downtime?
Expect 7–10 days of significant redness, swelling, & flaking. Total healing is different on each body part for example the face heals quicker because it has a quicker cell turnover than the legs or arms & darker skin in general takes longer to heal. -
How many sessions are needed?
Patients with darker skin often require an estimated of 6 sessions sometimes more or less because less-aggressive sessions rather than one heavy, intense treatment is needed to minimize trauma.
Results and Aftercare
- What is the post-care routine?
- Strict sun protection: SPF 50+ is mandatory.
- Gentle skincare: Use gentle cleaners and healing ointments (e.g., Aquaphor).
- Silicone Sheeting, Silicone Gels, Post Laser Care Products recommended by Jenny
- Avoid heat: No saunas, hot showers, or intense workouts immediately after.
-
Are the results permanent?
Results are permanent, as long as the recommended treatment plan is followed through in full.
When to Avoid
- Patients with active acne or infections should not get this treatment.
- Those prone to keloid formation should advise Jenny
- If you have a fresh tan, it is advised to wait until the skin has faded to its natural color to avoid PIH.
